Letters of Recommendation
Provide three letters of recommendation from individuals who can assess your abilities to succeed in the HSD Ph.D. Program, such as course instructors, research mentors, or internship directors. Statement of Research Interests and Goals
In one to two pages (single-spaced), describe why you wish to enroll in the HSD Ph.D. Program. Please include information on what areas of research interest you most, why you wish to pursue this research in the HSD Ph.D. Program at ASU, the HSD Ph.D. Program faculty members you hope to work with, and your goals for obtaining a degree. Curriculum Vitae or Résumé

HSD Academic Record Form
Summarize your coursework in the broad area of the human and social dimensions of science and technology on the academic record form. Upload the form into the online ASU Graduate Admissions Application. Please use Arial font.
Writing Sample
Submit a 10-25 page sample of academic writing, such as a course paper, an undergraduate thesis, or an excerpt from a master’s thesis. Upload the writing sample into the online ASU Graduate Admissions Application. Please use Arial font.
